How to Get Health Insurance Without a Job

Losing a job, leaving one, or simply living life outside traditional employment does not lock you out of health insurance. It just means you shop for it yourself instead of picking from an HR menu. Millions of Americans do exactly that. Here are the main roads, with the honest pros and cons of each.

The ACA marketplace

Marketplace plans accept everyone, no health questions asked, and cover pre-existing conditions from day one. If your household income is modest, subsidies can lower your premium substantially, and losing job-based coverage opens a 60-day special enrollment window so you do not have to wait for open enrollment. The honest downside: with the enhanced subsidies expired after 2025, unsubsidized 2026 premiums rose sharply, and the subsidy cliff at 400 percent of the federal poverty level is back. If your income lands above that line, you pay full freight.

Private underwritten plans

Private, medically underwritten PPO plans can be applied for any day of the year, with no enrollment windows at all. Because carriers review your health history before approving you, healthy applicants often see lower premiums and broader networks than unsubsidized marketplace options. The trade-off is real: these plans can decline applicants, so people with significant pre-existing conditions are usually better served on the marketplace, where acceptance is guaranteed.

COBRA, if you just left a job

COBRA lets you keep your old employer plan for up to 18 months, keeping your doctors and any deductible progress. You pay the full premium plus a small fee, which surprises most people. It shines when you are mid-treatment or late in a plan year; it is often the priciest option when you are healthy and starting fresh anyway.

Medicaid and other paths

If your income has dropped significantly, check Medicaid. Eligibility varies by state, but it exists for exactly these seasons of life, and enrollment is open year-round. Households with kids should also look at CHIP coverage for the children even if the parents choose another route. And if you are married, hopping onto a spouse's employer plan during their special enrollment window is often the simplest answer of all.

Whatever you do, avoid the gap

Going uncovered to save a few months of premium is the one move we caution everyone against. One emergency room visit can cost more than years of premiums, and getting covered after a diagnosis is much harder on the private side.

Timing matters more than most people realize, too. Special enrollment windows close 60 days after your coverage ends, and waiting until the last week leaves no room to compare plans or fix paperwork problems. Start shopping while your old coverage is still active if you can, so the new plan starts the day the old one stops and the gap never opens at all.
