Is Private Health Insurance Worth It? An Honest Look

You may have heard that private health insurance can cost less than marketplace coverage and wondered what the catch is. It is a fair question, and it deserves a straight answer. Private, medically underwritten plans are genuinely a better deal for some people and genuinely the wrong choice for others. Here is how to tell which camp you are in.

What private underwritten coverage is

These are individual and family plans sold outside the ACA marketplace. The word underwritten is the key: the carrier asks about your health history and decides whether to offer you a plan and at what price. Because the carrier knows the health of the people it accepts, it can price coverage for healthy applicants lower than a marketplace plan that must accept everyone at the same rate. Many are PPO plans with broad national networks, and you can apply for them any day of the year.

Where the value shows up

For applicants in good health, the savings versus unsubsidized marketplace premiums are often significant, and the gap widened when the enhanced subsidies expired at the end of 2025 and 2026 marketplace premiums rose sharply. Beyond price, people tend to like the network breadth, the year-round application window, and PPO access to specialists without referrals. For a healthy self-employed family paying full price, the comparison is frequently not close.

Where the honest catch lives

Underwriting cuts both ways. These plans can decline applicants, exclude or rate up certain conditions, and they are not required to match every marketplace benefit. If you have significant pre-existing conditions, ongoing treatments, or expensive prescriptions, a marketplace plan, which must accept you and cover those conditions, is usually the safer and often cheaper home. The same goes if your income qualifies you for strong subsidies: subsidized marketplace coverage is hard to beat, and we will tell you so.

Questions to ask before you buy

How does the plan handle your specific medications? What are the deductible, coinsurance, and out-of-pocket maximum in a worst-case year? Is your hospital in network? What happens at renewal time? A trustworthy advisor will answer all of these before asking for a signature.

The verdict

Worth it if you are healthy, paying full price, and want broad PPO access for less. Not worth it if underwriting would work against you or subsidies would work for you. Simple as that.

Renewal behavior is worth asking about too. Any plan can look good on day one; the better question is how a carrier has treated its existing members at renewal time over the years. An advisor who works with these carriers daily can tell you which ones have a steady track record and which ones price attractively up front and make up for it later.
